To Prepare
It is a great help to us to have your relevant medical history sent to us ahead of time. Please be sure to have any pertinent medical records from your primary care physician faxed to our office in advance. At least tell us your reason for coming. That way we can obtain any referral/pre-authorization as required by your insurance, pre-arrange necessary tests, and allow enough time for your doctor to see you.

To Bring
Your Insurance Card, Referral Form if applicable, any medical documentation, and all your medications in their original containers. Up to 3 family members or care-givers are welcome.

To Expect
You will need to arrive 15 minutes before your appointment. If you have not had the chance to fill out the paperwork in advance, please arrive 30 minutes early to give you time to complete the forms. We will ask you to complete our Registration Form, Short Medical History, and if appropriate, Chest Pain Form. Please also review and sign our Financial Policy Form and Notice of Privacy Form. We will mail you copies of these forms to complete ahead of time, or you can print them off this web site by clicking the appropriate links.

Allow a little over an hour for your visit. The doctor will interview and examine you, and generally give you recommendations at the end of your visit.

Directions to Free Parking
Behind the building, off California Drive, is a 3-storey parking structure with handicap access. Park on the 2nd level (B) and enter the building on that level. Proceed straight ahead. We are at the end of that hallway, nearest El Camino Real, on the left. There is also limited parking at the front of the building.

Insurance/Financial Policies
Please bring your insurance card(s) at the time of each visit and be prepared to make any co-payments. We are participating providers for Medicare, Mills-Peninsula Medical Group HMO’s, as well as many PPO plans. If you carry insurance through MPMG, you must have a valid MPMG referral prior to your appointment. We
accept MasterCard and Visa.
